Dear applicants!
Documents can be submitted by the holder or by another person who represents the holder’s interests (hereinafter - the Applicant) provided that he/she has an identity document.
The given list is relevant only for submission in the office. For remote submission, the list of documents is different.
To affix an apostille, the following must be submitted:
1. Originals of educational documents, requiring an apostille, of the template prescribed by law.
2. High-quality photocopies of each document without notarization or any certification.
3. Receipt confirming payment of the administrative fee in the amount of UAH 51.00 for each document for individuals and UAH 85.00 for each document for legal entities (download the invoice);
The document and its supplement are separate documents. Certificate on obtaining secondary education and supplement to the certificate are 2 documents, Diploma and diploma supplement are 2 documents.
4.In case of exemption from payment for services – the original and a copy of the document confirming the right to exemption from payment (for document holders).
The discount is granted upon presentation of the original document confirming the affiliation to the privileged category, and in the case of providing services regarding documents whose holders belong to the privileged categories.
5. Consent to the processing of personal data from the holder and the applicant (if they are different persons) of the approved form (download the form, download the form, template for filling out the consent).
The upper part of this consent is filled out and signed by the documents’ holder, the lower part is filled out and signed by the person who represents the holder’s interests (Applicant).
In case of a change of the last name and/or first name and/or patronymic of the document’s holder, a copy of the document on the change of the last name and/or first name and/or patronymic is attached.
In case of consent to the processing of personal data is provided by parents (guardians) of a minor child (under the age of 18), a copy of the child's birth certificate (certificate of establishment of guardianship over the child) is attached.
6. An application for affixing an apostille of the Ministry of Education and Science of Ukraine of the approved form (download the form, template for filling out the application).

7. Preliminary description of the order in the approved form (download the form, download the form, template for filling out the order description).
8. The package of documents is submitted in a transparent file.
Applications must be filled out and signed by the applicant (the person submitting the documents) in Ukrainian, clearly, legibly, without alterations - this information is used in the process of processing your documents.
To receive other services from the Centre (statement, translation, forming a package of documents for a foreign organization for recognition, etc.), the list of documents may be expanded.
For your convenience and to speed up the processing of your documents, please fill out an electronic application before your visit for submission through the Personal Cabinet. To do this, you need to go through a simple registration procedure on the website and log in to the “Personal Cabinet” section. In the Personal Cabinet, you can track the status of your documents until they are ready.
Payment for information and technical support services for verifying the authenticity of educational documents (in particular, for affixing an Apostille) is made by separate payment only AFTER the documents are submitted. Cost of services>>
The applicant is responsible for completing the package of documents, for the accuracy of the data provided, and for the authenticity of the documents.
