Please note! This list is relevant only for submission in the office. For remote submission, the list of documents is different.

To affix an apostille, the following must be submitted:

• originals of educational documents, requiring an apostille, in the form prescribed by law;
• high-quality photocopies of each document without notarization or any certification;
• a receipt confirming payment of the administrative fee in the amount of UAH 51.00 for each document for individuals and UAH 85.00 for each document for legal entities (download the invoice);
• in case of exemption from payment for services – the original and a copy of the document confirming the right to exemption from payment (for document holders);
• consent to the processing of personal data from the owner and the applicant (if they are different persons) of the approved form (download the form);
• an application for affixing an apostille of the Ministry of Education and Science of Ukraine of the approved form (download the form);
• preliminary description of the order in the approved form (download the form);
• the package of documents for each application is submitted in a transparent file;
• during the visit, the applicant is required to present his or her passport.

For your convenience and to speed up the processing of your documents, please fill out an electronic application before your visit through your Personal Cabinet. To do this, you need to go through a simple registration procedure on the website and log in to the “Personal Cabinet” section. In the Personal Cabinet, you can track the status of your documents until they are ready.
