In our Centre you may order several types of statements for embassy, organization for the recognition of qualifications or any foreign educational institution. According to the Resolution No. 924 of the Cabinet of Ministers of Ukraine "Issues on the National Information Centre of Academic Mobility" dated August 31, 2011, our Centre has the authority to check educational documents authenticity, providing information and explanation on educational system of Ukraine etc.
Statements are drawn up in the state language and on the letterhead of the State Enterprise "Information and Image Center" to be presented to whom it may concern.
Statement for apostille certification of information from the Unified State Electronic Database on Education (Statement of the IV category)
This statement will be required if you need to certify with an apostille “Excerpt from the Register of Educational Documents of the Unified State Electronic Database on Education regarding the document on education” or “Excerpt from the Unified State Electronic Database on Education regarding educational documents and information on the person's education” (hereinafter - Excerpt from USEDE).
The Apostille of the Ministry of Education and Science of Ukraine is affixed only on the originals of educational documents and certifies the signature and seal affixed to the document. The Excerpt from USEDE has the format of an electronic document, and its printout is a copy of such an electronic document, which cannot be certified by an apostille. The statement from the Center will confirm the information from the USEDE and will contain the Excerpt from USEDE as an annex. Such a statement together with the annex is certified by an apostille.
To obtain a statement, you need to provide an Excerpt from USEDE (in printed or electronic format) and consent to the personal data processing (from the owner and the applicant). You can order the statement at the same time as the apostille by registering the Personal Cabinet and creating an application for remote submission of documents or an application for submission of documents in our office. The time of statement issue – 2 or 10 working days. The cost is listed in the Cost of services section.
Statement "On Completed Education Verification and Rights" (statement of I and II category of difficulty)
Such statement may be issued by your educational institution. But if you are not able to apply there, we would like to help you.
A statement “On Completed Education Verification and Rights” (except medical, pharmaceutical and veterinary specialties can be provided by the National Information Centre of Academic Mobility) after the study verification procedure. You should submit a photocopy of educational document with its supplement, and the consent for processing of personal data, signed by the applicant and holder of documents. You may order such statement via Personal Cabinet or at the time of your personal visit to our office. The time of statement issue varies from 10 to 40 working days. The prices are mentioned in the section Cost of Services.
Statement "On the Assessment System in Ukraine" (statement of I and II category of difficulty)
At times to let you continue your education and recognize your qualifications, educational institutions ask for additional information about Ukraine’s assessment system due to the difference between the knowledge assessment standards in various countries at different periods of time. For this purpose, you will need a statement titled “On the Assessment System in Ukraine.” To obtain this statement, you need to submit a copy of your document on education with its Supplement and the consent to the processing of personal data (from the holder and the applicant). You may order this statement through the Personal Cabinet or at the time of your personal visit to our office. The statement can be issued for the period from 10 to 40 working days. Statement "On License and Accreditation Level of Higher Educational Institution" (statement of III category of difficulty)
To confirm Ukrainian qualification in foreign countries, you may need a statement titled “On License and Accreditation Level of Higher Educational Institution”. To obtain this statement, you have to submit a copy of your document on education with its supplement and the consent for personal data processing (signed by the holder and applicant). You may order this statement via Personal Cabinet or at the time of your personal visit to our office. The time needed to issue this statement is 40 working days. The prices are listed in the section Cost of Services.
Statement "Teacher Education Verification" (statement of III category of difficulty)
Such statement may be necessary if you have to prove your diploma validity; that you have never been deprived of it and have a right to work according to the obtained teacher qualification. In order to obtain this statement, you should submit:
- copies of your Diploma and its Supplement;
- notarized copies of all pages of your work record book;
- a copy of the name change certificate (in case of name change);
- the consent for personal data processing (signed by the holder and applicant);
- the name of organization, to which the statement is going to be submitted, should be specified, along with its address and registration number of the student (student ID)

Statement on impossibility to confirm documents on education
(Statement of the I category of difficulty)
Such a statement can be obtained for educational documents issued by educational institutions, the archives of which are located or were located in the zone of military actions (hostilities) or in territories that are under temporary occupation, in encirclement (blockade) due to the large-scale armed aggression of the russian federation against Ukraine, for which it is impossible to obtain confirmation of the fact of issue and other additional information required for affixing the Apostille of the Ministry of Education and Science of Ukraine to these documents (verification for further submission to a foreign organization).
The statement is issued on the basis of educational documents that correspond to the form and samples approved by the normative-legal acts of Ukraine in force on the day of their issue.
After analyzing the documents on education, the statement will indicate that the documents meet all the requirements of the standards and instructions for their drawing up that were in force in Ukraine on the day of their issue, information on the compliance of the signature and seal (in the presence of such) which the document bears with the sample, and also there will be stated the reasons for the absence of the apostille on the educational document and the request not to interpret the absence of the apostille as a fact that testifies against its holder.

If necessary, this statement can be certified by the Apostille of the Ministry of Education and Science of Ukraine.
